1. Open Fathom2.
2. Click Object, New, Collection. To name the collection, double click on the collection icon in the window.
3. Now click the Collection menu and choose Create Survey.
a. Click the survey tab.
b. Click on top of <new> under the Attribute column heading. Type in the title for the first survey question. This is what will appear as the column heading in the table. This title should be as concise yet descriptive as possible. This will allow you to remember the question you are asking without having to look it up.
c. Press tab, and type in the first survey question in the second column.
d. Click on top of <new> in the second row under the Attribute column heading. Type the title for the second survey question.
e. Press tab and type in the second survey question in the Question column.
f. Repeat until all attributes and questions have been entered.
4. If you would like for any of your questions to have a drop down menu with choices for an answer, such as male and female for gender, click the categories tab.
a. Click <new> under Set in the first column and type in the title you would like to appear.
b. Click tab and type in the different options you would like to appear in the categories column, separating each with a comma (no spaces are required between comma and next attribute). The survey defaults to have the first option as the answer if no answer is chosen. It is helpful to make your first choice "no answer".
5. If you would like a measurement value to automatically appear in the results, such as centimeters when measuring leaf length, click on the label for the question.
a. In the units box type in the type of units you would like Fathom2 to use.
Repeat this process until all units have been recorded.
6. Now that the survey is complete, click the survey tab. Type your username in the username box at the bottom right hand corner. Click the Upload Survey button in the bottom left hand corner. Fathom will ask for your password. Enter your password and click OK.
